ELECTRONIC CONSENT FORMS
For convenience and accessibility, I use HIPAA-compliant electronic systems to send intake and consent forms.
Private-pay clients:
I use a HIPPA-compliant electronic health record system called SimplePractice to send intake and consent forms.
If you prefer to comple forms in hard copy, that can also be arranged. Please let me know your preference when you reach out.
SUPERBILLS
For private-pay clients using out-of-network benefits, I can provide monthly super bills upon request.
A superbill is a receipt that includes the diagnosis, and billing codes required by insurance companies for reimbursement. Reimbursement amounts vary depending on your insurance plan and whether you have met your deductable.
Before submitting super bills, you may want to ask your insurance provider:
-
What is the reimbursement rate for the CPT code 90837 (individual therapy, 53 - 60 minutes)?
-
What is the allowable amount for CPT code 90837?
